LIABILITY RELEASE FORM

I realize that participation in online dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I assume all risks related to the use of any and all spaces used by I the participant, including spaces located outside of Canada.

I agree to release and hold harmless Daphné Paquette, including its dancers from any cause of action, claims, or demands now and in the future. I will not hold Daphné Paquette liable for any personal injury or any personal property damage, which may occur on my home premise before, during or after classes. Furthermore, I agree to obey the class rules and take full responsibility for my behaviour in addition to any damage I may cause to myself or my space during an online class session.

I understand that Daphné Paquette is insured. In the event that I should observe any unsafe conduct or conditions before, during or after my classes, I agree to report the unsafe conduct or conditions to the Artistic Director as soon as possible.

I grant Daphné Paquette permission to use photos and video of me, taken within class settings on the Daphné Paquette website at www.daphnepaquette.com and for any promotional material and social media websites related to Daphné Paquette.  I understand that these images will not be given or sold to any outside individual or organization.

I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
